  • You Must Send the requested paperwork to support@theperfectrx.com. Our Licensed Mental Health providers will review the documents. If our providers are able to complete the paperwork they will email you back the completed forms. If the documents can not be completed, the documents will be sent back to you and you will be refunded the cost of your package.
     

  • If you to are not approved for whatever reason, you will receive a full refund within 7-10 business days minus a $50 Provider Assessment fee. Please email us at support@theperfectrx.com if this issue occurs.
